Please download the new patient form, read through it carefully, and provide accurate and complete information. This will maximize your useful time with the Psychiatrist and this information is used by the Psychiatrist to make treatment decisions.

Please include a copy of your insurance and ID card with the completed forms.

About Dr Rungta

I am a American Board of Psychiatry & Neurology (ABPN) certified Psychiatrist. I earned my MD degree from Wayne State University School of Medicine. After that I attended University of Cincinnati Medical Center for training in General Psychiatry, Following this I continued my training and further specialized in Child & Adolescent Psychiatry at University of Toledo Medical Center. Currently I am the Medical Director for Center for Childrens Integrated Services at a CMHC (Community Mental Health Center), while also holding the position of Clinical Assistant Professor with Michigan State University - College of Human Medicine.
